The PPO Essential KeyCare 500 has a monthly premium of $448.00 and there is out of network coverage.  The annual out of pocket deductible is between $500-$1000 and there is 30% coinsurance.  For primary care and specialist care there is $30 copay for visits 1-3 and 30% copay after deductible for all other services.  For annual physicals there is 30% copay.  For impatient and outpatient services as well as emergency room care, there is 30% copay after the deductible.  There is also optional prescription drug coverage, supplemental accident and preventive care.

The Lumenos HSA 1500 has a monthly premium of $315.00 and there is out of network coverage.  The annual out of pocket deductible is between $3,000 and there is 20% coinsurance.  For primary care and specialist care there is 20% copay after deductible is paid.  For annual physicals there is no charge.  For impatient and outpatient services as well as emergency room care, there is 20% copay after the deductible.  There is also prescription drug coverage and dental and maternity coverage is optional.  A Health Savings Account is also included.

The PPO Essential KeyCare 500 has a monthly premium of $273.00 and there is out of network coverage.  The annual out of pocket deductible is between $2,500/$5,000 and there is 30% coinsurance.  For primary care and specialist care there is $30 copay for visits 1-3 and 30% copay after deductible for all other services.  For annual physicals there is 30% copay.  For impatient and outpatient services as well as emergency room care, there is 30% copay after the deductible.  There is also prescription drug coverage, and dental, preventative care and supplemental accident coverage is optional.
