The Network Copay Select plan’s monthly premium is $139.13. There is a $2,500 out-of-pocket deductible cost. For primary and specialist care there is $25 copay. The HSA 100 plan’s monthly premium is $81.17. There is a $2,900 out-of-pocket deductible cost. For primary and specialist care there is no additional charge after deductible. For annual OB/GYN and annual physical examinations, there is a also no additional charge after deductible. For inpatient and outpatient visits there is no additional charge after deductible. For drug coverage, there is no charge for drugs after deductible is paid for and there is no limit for maximum annual benefit.
For individuals, Golden Rule’s monthly premium ranges from $64 to $249. For families, Golden Rule’s monthly premium ranges from $227 to $995.
