Preferred Provider Organization plans allow members to choose from out-of-network primary care physicians, specialists and hospitals.  The copay is usually higher when choosing out-of-network.  The PPO Monogram Total Plus plan has a high deductible but a low premium.  This type of plan is suggested for healthy individuals who visit doctors regularly to take preventive care.
