The Portrait Share 80 Plus Rx PPO monthly premium is $134.73.  There is a $2,500 out-of-pocket deductible cost.  For primary care there is $35 copayment and $50 copayment for specialist care.  For annual OB/GYN and annual physical examinations, there is 20% copayment.
