With Tonik’s Preferred Provider plans, members are free to choose a doctor or other provider out of Tonik’s network, however the patient will pay more for the visit and/or care.

The Tonik 1500 plan is for the “calculated risk-taker.”  Within this plan, there is a $40 copay for doctor visits, and a $1,500 deductible.  The monthly premium is $134.19 per month for people between the ages of 19-29.  In the case of emergencies, the member will pay a flat rate of $100 per visit.  Tonik pays for a $50 routine eye exam, regular glasses, contact lenses or sunglasses.  There is a $25 deductible for dentist visits and a $0 deductible for preventive and diagnostic care regarding dental work.  Rx covers generic prescription drugs, and members pay $10 for 30-day supply from the pharmacy and $20 for 60-day supply through mail order.  In-network Annual Out of Pocket (OOP) Maximum is $1,500.

The Tonik 3000 plan is for the “part-time daredevil.”  Within this plan, there is a $30 copay for doctor office visits, and a $3,000 deductible.  The monthly premium is $82.93 per month for people between the ages of 19-29.  In the case of emergencies, the member will pay a flat rate of $100 per visit.  Tonik pays for a $50 routine eye exam, regular glasses, contact lenses or sunglasses.  There is a $25 deductible for dentist visits and a $0 deductible for preventive and diagnostic care regarding dental work.  Rx covers generic prescription drugs, and members pay $10 for 30-day supply from the pharmacy and $20 for 60-day supply through mail order.  In-network Annual Out of Pocket (OOP) Maximum is $3,000.
