SuperBlue Plus- This plan is a preferred provider organization (PPO) with annual deductible options that range from $500 to $5,000 for individuals and $1,000 to $10,000 for families. In network office visits are subject to a $25 copay and emergency care is subject to a $100 ER copay. Prescription drugs are provided through a preferred pharmacy network and subject to 50% coinsurance with no deductibles.
Limited Benefit Plan- This plan has an annual deductible of $250 for individuals and $500 for families. The lifetime maxiumum benefit is unlimited and the annual maxiumum benefit is $25,000 per covered person. Primary Care Physician office visits are subject to a $15 copay per office visit and specialist visits are subject to $30 per office visits. Prescription drugs are provided through a preferred pharmacy network and generic drugs are subject to a 50% copay.
High Deductible Health Plans- There are two deductible options for HDHPs:
- HDHP Direct Pay $3,000/$6,000- This plan has an annual deductible of $3,000 for individuals and $6,000 for families with a lifetime maxiumum benefit of $2,000,000 per covered person. The first $300 of preventive care services such as annual gynecological exam, routine Pap smear and routine HPV testing are paid at 100% then subject to deductible and 100% thereafter. Prescription drugs are subject to deductible then 50%.
- HDHP Direct Pay $5,000/$10,000- This plan has an annual deductible of $5,000 for individuals and $10,000 for families with a lifetime maxiumum benefit of $2,000,000 per covered person. The first $300 of preventive care services such as annual gynecological exam, routine Pap smear, and routine HPV testing are paid at 100% then subject to deductible and 100% thereafter. Prescription drugs are subject to deductible then 50%.
