Keystone Health Plan East HMO- This plan is a Health Maintenance Organization with annual deductible options of $1,500, $2,500, and $5,000 and copay plan options of $10, $15, and $20. Office visit copays range from $10-$30 and specialist visit copays range from $20-$50 depending on the plan that is chosen. Prescription drug coverage have copays of $15 for generic drugs, $35 for brand formulary drugs, and $35 for non-formulary drugs after a prescription deductible is met. Monthly premium rates depend on age, health condition, and plan option chosen. A sample rate for a 30 year old woman ranges from $141 to $287.
Personal Choice PPO- This plan is a Preferred Provider Organization (PPO) that has annual deductible options of $300 and $500. Coinsurance ranges from 80% to 100% and out of pocket maximums range from $0 to $2,000. Office visits have copays that range from $10-$15 and prescription drug coverage is subject to either an 80% or 60% coinsurance.
